By accessing or using the MRI Pass platform ("Service"), operated by MRI Pass ("we," "us," or "our"), you agree to be bound by these Terms and Conditions. If you do not agree to these terms, do not use the Service.

2. Description of Service

MRI Pass is a digital MRI safety screening platform that allows patients to complete safety questionnaires, disclose implants and medical devices, and receive safety clearance levels from certified Patient Safety Officers (PSOs). The Service is intended for use by patients, healthcare facility technologists, PSOs, and facility leadership.

3. Eligibility and Registration

Registration is available only through invitation links provided by participating healthcare facilities. By registering, you represent that:

• You are at least 18 years old or have parental/guardian consent.
• The information you provide is accurate and complete.
• You will maintain the security of your account credentials.

4. Accuracy of Information

You are responsible for ensuring that all information submitted through the Service, including but not limited to implant disclosures, medical history, and safety questionnaire responses, is truthful, accurate, and complete. Inaccurate or incomplete information may result in an incorrect safety clearance and could pose serious health risks during MRI procedures.

5. Not a Substitute for Medical Advice

The Service is a screening and record-keeping tool. It does not provide medical advice, diagnoses, or treatment recommendations. All safety clearance decisions are made by qualified Patient Safety Officers and healthcare professionals. Always follow the guidance of your healthcare provider regarding MRI safety.

6. Account Security

You are responsible for maintaining the confidentiality of your login credentials and for all activity that occurs under your account. You agree to notify us immediately of any unauthorized use of your account. We are not liable for any loss arising from unauthorized access to your account.

7. Intellectual Property

All content, features, and functionality of the Service, including text, graphics, logos, and software, are the exclusive property of MRI Pass and are protected by applicable intellectual property laws. You may not copy, modify, distribute, or create derivative works based on any part of the Service without prior written consent.

8. Prohibited Use

You agree not to:

• Use the Service for any unlawful purpose.
• Attempt to gain unauthorized access to any part of the Service.
• Submit false or misleading information.
• Interfere with or disrupt the Service or its infrastructure.
• Use the Service on behalf of another person without their consent.

9. Limitation of Liability

To the fullest extent permitted by law, MRI Pass and its officers, directors, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of or inability to use the Service. Our total liability shall not exceed the amount you paid, if any, for use of the Service.

10. Disclaimer of Warranties

The Service is provided "as is" and "as available" without warranties of any kind, either express or implied, including but not limited to implied warranties of merchantability, fitness for a particular purpose, and non-infringement.

11. Termination

We reserve the right to suspend or terminate your access to the Service at any time, with or without cause, and with or without notice. Upon termination, your right to use the Service will immediately cease.

12. Changes to Terms

We may update these Terms and Conditions from time to time. Changes will be posted on this page with an updated effective date. Your continued use of the Service after any changes constitutes acceptance of the revised terms.

13. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of Arizona, without regard to its conflict of law provisions.
